Creatinine is one of those words I still want to confuse with creatine.
And honestly, I know I am not the only one.
Creatine and creatine both sound like gym culture. Protein tubs. Men in sleeveless shirts explaining macros with the confidence of a podcast host in a backwards hat.
Are they related? Technically, yes, in the way that medical words love to be related just enough to make patients feel stupid. Creatine is involved in muscle energy. Creatinine is a waste product that comes from normal muscle metabolism and gets cleared by the kidneys.
The way I memorize it:
Creatine is more of the input/energy conversation.
Creatinine is the leftover trash your kidneys are supposed to take out.
And in cirrhosis, trash removal becomes a very big deal.
Because you learn quickly that the kidneys are often dragged into the liver’s bad decisions.Sometimes they are collateral damage. Sometimes they are early warning witnesses. Sometimes they become active participants in the disaster. Either way, the liver and kidneys are far more connected than I understood before I got sick.
Which is why creatinine gets top billing.
It is not technically a liver enzyme. It is not “made by the liver” in the way albumin is. But it still earns a seat at the big-kid table because kidney function matters enormously in advanced liver disease.
Creatinine is part of MELD.
It is one of those numbers doctors watch closely because when kidney function starts changing in cirrhosis, the whole picture can shift quickly.
And that is what makes it so strange as a patient.
You think you are here for liver disease, and suddenly the kidneys are walking into the room like uninvited but legally necessary co-stars.
For me, I have been incredibly fortunate so far. My kidneys have been resilient, which is honestly shocking because kidney disease is the one health issues that runs through my family. So while my liver has clearly been out here submitting out of office requests, my kidneys have mostly been showing up to work with their little lunchbox, doing the job.
But creatinine still fascinates me because it is not as simple or static as I first assumed.
Creatinine is not just “high bad, low good” in some cartoonishly easy way. Muscle mass matters. Hydration can matter. Medications can matter. Kidney function matters. Trends matter. And because creatinine comes from muscle metabolism, a person with lower muscle mass may have a lower creatinine even when their kidneys are not necessarily as perfect as the number looks at first glance.
That part really matters in cirrhosis, because muscle wasting is common.
So when I see a low creatinine, I am grateful, yes.
But I also understand now that this number has layers.
It is a kidney marker, but it lives inside a much bigger story: muscle, filtration, hydration, medication, sex-based reference ranges, liver disease severity, and the very complicated partnership between organs that were apparently in a group project I did not know I had been assigned to.
Creatinine may not be the flashiest lab on the page. But in cirrhosis, it is one of the ones you learn to respect very quickly.
